Overview

This position is responsible for the delivery of resource assistance and guidance to families of patients being treated at Dana-Farber Cancer Institute. This includes delivery of concrete resource services such as short-term accommodations, parking, and transportation, referrals to appropriate local, state, and federal assistance programs as well as help with applications for assistance to non-profit foundations. This position will work directly with patients and their families in both inpatient and outpatient settings.

Requirements

  1. May meet with newly diagnosed families in inpatient and outpatient settings and conducts assessments of families' financial status/eligibility for concrete resource assistance and documents in electronic medical record.
  2. Conducts assessments of families' financial status/eligibility for concrete resource assistance. Responsible for making timely direct referrals, arranging concrete resources such as short-term accommodations, parking and transportation. As member of multidisciplinary team, assists patients in securing assistance through skillful matching of needs to available resources.
  3. Refers families to assistance and appropriate resources both within the hospital and through agencies, programs and foundations in the community. Serves as patient/family advocate in negotiating both internal and external systems and serves as liaison between hospitals and the resource community. 
  4. Makes timely referrals to community agencies, as appropriate, and coordinates and communicates regarding a wide range of social agencies such as schools, MassHealth, the American Cancer Society, and temporary housing options. 
  5. Responsible for detailed record keeping which may include the following: statistical reports, progress notes in the patient's medical record, intake record of patient contact, and correspondence with community resources. 
  6. Follows eligibility guidelines and rules with respect to assistance granted from internal resources. Responsible for consistently executing internal and external scans of the environment to research and track new and available concrete resources for patients and families. Also maintains current data on available resources.
  7. Communicates resource specific information to patient and family caseload. Directly assists those patients not followed by a social worker.
  8. May coordinate and lead resource information update sessions on his/her disease centers for Care Coordination staff, and, as requested, for other departments. 
  9. May be responsible for special projects and/or pilots, focused work teams or efforts, and or responsible for serving on a committee relevant to the Resource Specialist role (e.g. GCSF donor short-term accommodations program, monthly short-term accommodations' statistical record keeping, solid tumor pharmacy resource assistance, etc). 
  10. Covers for other Resource Specialists within the Department as needed. 

Qualifications

  • Bachelor's degree or 5+ years relevant experience. Undergraduate experience may count towards work experience. Fluency in Spanish preferred.
  • High customer service orientation. Strong organizational and interpersonal skills, attention to detail and record keeping, and ability to work independently are essential. Must be skilled in relating to a multidisciplinary team and able to promote and engage in relationship-building. 
  • Ability to communicate effectively both orally - in person and on the phone - and in writing and provide empathy in difficult interpersonal situations. 
  • Ability to prioritize and work in a fast-paced environment. 
  • Ability to interface with adults and children ranging in age from newborns to young adults. 
  • Able to multi-task and capable of problem-solving complex issues through both creative problem-solving and strong analytical skills on a daily basis. 
  • Must be proficient in online research, email, and MS Office, particularly Excel and Word.
