Overview

The Patient Care Coordinator Supervisor is responsible for overseeing the daily operations and management of a portion of the patient care coordination team (6-8 team members). This role ensures that patient care activities are efficiently organized and executed, facilitating seamless communication between patients, families, and healthcare providers. The Supervisor plays a critical role in maintaining high standards of patient care and satisfaction, ensuring compliance with institutional policies and procedures, and supporting the team in delivering compassionate and comprehensive care. The position requires strong leadership skills to manage a team of coordinators and to foster a collaborative environment that promotes excellence in patient care.

The Patient Care Coordinator Supervisor will stay current with trends and developments in patient care coordination and serve as a role model and mentor to the PCC team. They will develop and maintain effective working relationships with healthcare providers, external stakeholders, and other health-related resources, and actively participate in departmental and interdepartmental committees.

Additionally, the Patient Care Coordinator Supervisor will manage reports for the Quality and Clinical Management Committee (QCMC), including data quality analysis, and assist the pharmacy manager in planning and implementing quality improvement initiatives and/or plans of correction when needed. They are expected to provide staffing coverage and support the team as necessary.

  • Supervise and manage the patient care coordination team, ensuring efficient workflow and high-quality patient care.
  • Develop and implement policies and procedures to enhance patient care coordination and improve patient satisfaction.
  • Monitor and evaluate team performance, providing coaching and feedback to ensure professional growth and development.
  • Collaborate with healthcare providers and departments to ensure seamless communication and coordination of patient care activities.
  • Address and resolve patient and family concerns, ensuring a compassionate and responsive approach to patient care.
  • Oversee scheduling and resource allocation to ensure adequate coverage and support for patient care activities.
  • Conduct regular team meetings to discuss goals, challenges, and opportunities for improvement in patient care coordination.
  • The PCC supervisor will be expected to fill in about 50% of the time. The balance of the time will be committed to management of the team with some coaching and support by the manager.

Supervisory Responsibilities

Directly supervises at least two staff. Hires, develops, and manages staff to achieve organizational goals. Sets clear expectations, delivers feedback, and monitors performance for quality, efficiency, and compliance with policies and procedures. Mentors staff, fosters career growth, and cultivates a positive and productive work environment.

Education Required

High School or GED required

Experience Required

Five years of experience in a retail, outpatient, and/or specialty pharmacy, oncology, healthcare, or health insurance environment is required.

Five years of experience as a Patient Care Coordinator or equivalent role is required.

Experience with Therigy and/or EnterpriseRx (or equivalent systems) is required.

Experience Preferred

Experience in other pharmacy technician roles such as retail production, utilization review or Prior Authorization is highly preferred.

At least one year of supervisory or leadership experience is preferred.

License/Certification/Registration Required

Massachusetts Board of Pharmacy Registration as Pharmacy Technician or Intern required.  Nationally Certified Pharmacy Technician license required.

  • Strong leadership and management skills with the ability to motivate and develop a team.
  • Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare providers.
  • Ability to manage multiple priorities and work effectively in a fast-paced environment.
  • Strong problem-solving skills and attention to detail.
  • Knowledge of healthcare systems and patient care coordination processes.
  • Proficiency in healthcare management software and tools.
